I understand that contractors must be at least 18 years old and legally able to provide services in the United States.

I understand this opportunity does not guarantee work, clients, assignments, or income. I am responsible for all taxes and for following HIPAA, confidentiality rules, and all state and federal laws. I am responsible for holding the proper license or credential and keeping it active.
I understand that company policies may change and that I am responsible for reviewing updates. I am responsible for my own equipment, internet, and workspace.
I understand that I am not considered a contractor for Empowered Harmony Counseling, Psychiatry & Wellness Center until I submit a signed contract and complete all onboarding steps. I cannot represent myself as affiliated with the company until this process is complete.
